According to "git blame", hundreds of lines in the ref manual were
commented out back in 2015 and have just been taking up space since
then, so get rid of them. Where they were commented out:

    ref-manual: Backed out the bits for the context PNG images.

    We did not have time to do this for 1.8.  So, I restored the look
    of the glossary pre-images.  I did leave commented code in both
    the glossary chapter and the style sheet for later.

  as i'm going to be messing with the ref manual, i see no reason to
keep dragging around hundreds of five-year-old comments.
